Summary
Delirium is a serious neuropsychiatric condition characterized by an acute change in cognition and attention that affects a significant proportion of hospitalized older adults and is associated with significant morbidity and mortality. Prevention of delirium is an important part of the care of hospitalized older adults. The Hospital Elder Life Program is a multicomponent intervention that has been shown to reduce the incidence of delirium. As many cases of delirium are overlooked, its diagnosis is important and can be achieved using the Confusion Assessment Method, which relies on four cardinal features of delirium: acute onset, inattention, altered level of consciousness, and disorganized thinking. The etiology of delirium is often multifactorial with contributions from predisposing factors (such as sensory impairment, chronic illness, and cognitive impairment) and precipitating factors (such as infection, polypharmacy, or illness). Once diagnosed, delirium should be evaluated with a thorough history, complete physical, medication review, and targeted tests in an effort to identify these factors. Management should focus on addressing the noted precipitating and predisposing factors with limited use of low-dose antipsychotic medications in patients at risk of self-harm.

Summary
Growing evidence supports the involvement of EF in academic performance and, consequently, EdF in learning disorders (commonly known as learning disabilities or LD). Often, the first indicator of LD is low academic achievement in areas of reading, math, or writing. The terminology used to describe different LD conditions varies by field and includes dyslexia and reading disorder/disability, dyscalculia and mathematics disorder/disability, and disorder of written expression/writing disability. Comorbidity among these learning disorders is common, as is the co-occurrence of LD with ADHD.
Recent US data indicate that almost 2.5 million public school students received special education and related services for LD in 2008, and 43% of all public school students receiving special education and related services were identified as having a specific learning disability. Federal guidelines used in educational settings indicate that a child may have a specific LD if, in the absence of any other disability or limiting condition and despite appropriate instruction and experiences, the child does not adequately achieve for age or state standards in one of the following eight academic areas: oral expression, listening comprehension, written expression, basic reading skills, reading fluency skills, reading comprehension, mathematics calculation, or mathematics problem solving. The DSM-IV-TR indicates diagnosis of specific disorders of reading, math, and written expression based on lower than expected performance on standardized tests “given the person’s chronological age, measured intelligence, and age-appropriate education.” The proposed changes for DSM-5 include defining these disorders based on low performance on standardized measures without requiring a significant discrepancy between measured intelligence and achievement.

Nutrigenomics is the study of how constituents of the diet interact with genes, and their products, to alter phenotype and, conversely, how genes and their products metabolise these constituents into nutrients, antinutrients, and bioactive compounds. Results from molecular and genetic epidemiological studies indicate that dietary unbalance can alter gene–nutrient interactions in ways that increase the risk of developing chronic disease. The interplay of human genetic variation and environmental factors will make identifying causative genes and nutrients a formidable, but not intractable, challenge. We provide specific recommendations for how to best meet this challenge and discuss the need for new methodologies and the use of comprehensive analyses of nutrient–genotype interactions involving large and diverse populations. The objective of the present paper is to stimulate discourse and collaboration among nutrigenomic researchers and stakeholders, a process that will lead to an increase in global health and wellness by reducing health disparities in developed and developing countries.

Of the many important developments in space astronomy during the past three years perhaps the most dramatic was the voyage of the Mariner II which passed less than 35 000 km from the surface of the planet Venus on 14 December 1962 and from which valuable measurements were made of the mass and magnetic field of Venus of the structure of its atmosphere and of conditions in the interplanetary medium. A second event of great importance to solar physics was the successful launching in March 1962 of the first of NASA’s Orbiting Solar Observatories’ the first satellite to be especially designed as a stabilized platform for solar observation. Six additional satellites in this series are planned between 1964 and 1966. Striking advances in the range and quality of rocket solar spectra have also been achieved. Soft X-ray spectra have been photographed to a short-wave limit of 13·8Å and the resolving power at longer wavelengths has been improved by as much as a factor of 10.
